Concept Paper: Outline

Your Concept Paper: Outline is due at the end of this week. Below are some guidelines that I hope you find helpful. Remember to follow the grading rubric as I will utilize the grading rubric. Please let me know if you have questions. I will be online every day this week to assist with this project as this assignment will shape your final paper due in Week 8. Post any questions you may have in the Q&A section or e-mail me. When preparing the concept outline and final paper remember this is a proposal assignment – so the final product should look like a proposal about your health project. The goal of the end product is to develop a proposal that you could provide to someone to elicit funding or support for the planned project. To get an ideal regarding what a project proposal looks like conduct an internet search to see the types of programs are out there to address your issue. So the final product is not a research style paper but rather a formal proposal that includes the required elements outlined below.

Cover Letter: This letter should be a letter written to inform a funding agency about your project with the goal of soliciting funding. In the letter touch up (briefly) the key pieces of information below.

• Project Management
• Evaluation

Cover Sheet: title page
Goals: need to state a project goal that identifies the population and problem. The goal must be measurable. For example: To educate expectancy mothers about the benefits of breast feeding.

Statement of Need: support the need for the program by describing the significance of the issue – for example: how many people impacted, resulting physical and financial costs, and projections for future severity – will the problem continue to get worse. The goal is to demonstrate that an issue exists that demands a solution to address (need to justify your cause).
Impact: who will be impacted by the project: the target population, the community ……….Identify who and the projected impact.
Sustainability: discuss how the project could be supported long term – what revenue sources would be needed to support program and how will you obtain?
Budget: how much the project will cost to develop, implement, and sustain?
Project Management: how would the project be managed. For example, a program director and an assistant program director.
Evaluation: need an evaluation method to measure the impact of the project. The evaluation should be based on your objectives- each objective should have a means of evaluation.
Enclosures: materials that will be used for the project.
Use the provided template to complete the evaluation matrix. Just fill in the required information.
The key with this project is to have a clearly defined goal and to describe the steps (through objectives) to identify how the goal will be accomplished and measured to evaluate the impact (to determine if you met your goal).
