Health Promotion Plan Development.

The major focus of this course is increased skill in the practice of health promotion. Therefore, students must have the opportunity to engage in an extended project related to health promotion. Students will be responsible for planning and carrying out a health promotion plan or “prescription”. Pender et al Chapter 5 is a primary resource for developing the health promotion plan.

Students will use the self-assessment data from the previous assignments (personal health genogram and personal health practices) to develop a health promotion plan. The plan begins with goal setting based on the assessments completed. The student will select one of these goals and develop a plan to ameliorate the health risk. The plan will be shared with a peer who will serve as a health coach while the plan is implemented for the following three weeks. Reassessment and adjustments in the plan may occur during that 4 week period. Finally, evaluation of the plan and its success will occur.
