Family Assessment, Wellness and Health Promotion Paper
Welcome to your first “big” assignment in this course! We would like you to assess a family (no relatives). Since many illnesses have an geneticist orientation and/or involve family lifestyle behavior changes, viewing health and wellness within an objective family context is critical. The Informed Consent is a legal accountability document that needs to be obtained on your FIRST FAMILY VISIT. It needs to be signed by the primary member (key informant) in your family (over 18 years of age), and it needs to be signed by you. Please scan and return with signatures to your instructor via BB mail attachment. Consents NOT received within 2 weeks of visiting your family may receive a 0 grade for this assignment. (See informed consent).
Your book “Nurses and families; A guide to family assessment and intervention” will be your primary resource for conducting your interviews, and developing components of your Family paper- such as the family genogram and ecomap. The authors’ Calgary Family Assessment and Intervention Model is used as the primary organizing model to assist you in conducting your home visits and organizing your assessment and intervention data.
Many students use various software to create a genogram and ecomap e.g. Mac, Word, or use a PPT slide. Hand-written maps are not accepted.
Students should also use The Community Guide, HP 2020 websites and this important Periodic Screening Guide as helpful HP data collection tools.
*a fictitious family will need to be made; I currently live in San Antonio, Texas. This is where the family needs to “Reside”.
