The Action Plan Brief Paper should address the following:
– Two pages, single-spaced (1 page of content, 1/2 page introduction, 1/2 page conclusions and recommendations)
– Create a plan for going forward with addressing this health issue in the community
– Plan for a community assessment and how recommendations from this will be implemented (community assessment will take place in next briefer)
– Identify the scope at which you will look at the problem (neighborhood, town, community, senior center, hospital, state-wide, etc.)
– Identify the population and expectations
– Identify and analyze the amount or type of financial backing that will be provided
– Identify a reasonable timeline
– Make note of evaluation or monitoring within the program (can be included in conclusion)
