By Week 5, Day 5, respond to any one of the three exam questions assigned by your facilitator. Make sure you use the APA 6th edition style guidelines. Reference from 2010 to present.
1. A 16-year-old male patient presents with acute onset of right testicular pain. He is nauseated, but is not vomiting, and has a temperature of 101 orally. He denies recent trauma or illness. On physical examination, you note that he has one descended testicle that is red, swollen, and tender to palpation. The cremasteric reflex is absent.
Determine this patient’s diagnoses. What is your immediate intervention and what is your future plan for work up? Is this patient at risk for any complications in the future? What is the meaning of the absent cremasteric reflex?
2. A 30-year-old female patient presents with lower abdominal pain that began two days ago. The pain has increased and she is experiencing vaginal bleeding that is scanty. Her last normal menstrual period ended two weeks ago and was normal for her, lasting four days. She is a Gravida 2, Para -0, Miscarriage -2, and has been attempting to conceive with her husband. She denies dysuria or urinary frequency.
Determine three differential diagnoses for this case. Your rationale for including these differentials should be based upon the patient’s age, history, and presenting symptoms. For each of the differentials, explain what you would typically find on physical examination.
3. A 28-year-old female presents with complaints of right breast tenderness since the last one week. She states that she has experienced this in the past just before her menstrual cycle begins. On physical examination, you note that both breasts have palpable soft, pliable, movable lumps throughout. However, on the right breast, there is a nontender, fixed solitary nodule, approximately half a centimeter.
Discuss your diagnoses for this patient and your interventions. Explain the pathophysiology of these two diseases and how you would explain them in terms that patients can understand.
