Planning for Transition: Models to Consider

A transition plan is to be incorporated into the annul IEP no later than when the student will turn 16 years old. If a student serviced under IDEA will turn 16 during the year of the current IEP, a transition plan is required. Your text provides examples of several models (PATH, MAPS, and PCP) and strategies in which to approach planning for transition and the long-term possibilities for a student eligible for special education services.
For each of these models, provide a summary of how the model:
Ensures FAPE;
Highlights and supports the student’s vision and wishes;
Incorporates the voice of key stakeholders;
Utilized assessment data to develop goals;
Frames development of the transition plan related to:
Vocational, employment, or post-school education;
Related services;
Independent living; and
Community services/safety access.
