report on assessment and Return to work suitable duties plan. The work is essential for clinical practice in the area.

My case study is a person with low back pain who works in a cashier cafeteria

You need to make up all the relevant information to fill in all of the headings for example you can make her a 55 year old who works in a starbucks café who makes coffee all day or sandwiches and dicuss what is involved it is up to please fill in all the headings and use the many examples that I have attached as a guidance.

Task

You will be allocated a “client” (fictitious) who works in specific type of job.

1. MY CASE STUDY IS A PERSON WITH Low back pain WHO WORKS IN THE Cashier in the cafeteria

You will analyse the work

tasks and demands of the four types of jobs in week 2. On the basis of your findings and the given

information on your “client”, you will complete the worksite assessment report (Part A) and the RTW

suitable duties plan (Part B).

Part A: Injured worker’s worksite inital assessment report form will be uploaded (30%) – 1000 to 1500 words

You will analyse the job of the “client” focusing on the risk factors most relevant to your ‘client’ and

submit a worksite assessment report for your client (Worksite Assessment Report sample will be put

in the attached files to be used to complete ).

Part B: RTW suitable duties plan form will be uploaded (30%) – 800 to 1,000 words

With the case study information and the functional assessment information provided, and the type of

job assigned, you are to develop a RTW suitable duties plan for your client (a RTW – suitable duties

plan will be put in the attached documents to be used).

a) Develop 3 stages of a suitable duties program for your client:

Include:

• Tasks selected, how these are to be modified (e.g order of duties, hours, time of the

day).

• Any modifications to method for completing the task, routine, order of duties, the

workstation layout etc (be careful to be realistic).

• The review points in the plan. When will these occur and what will happen at these

times (e.g phone contact/workplace review/discussion with supervisors etc).

The first stage will line up with the information provided by the medical certificate. Stages 2 and 3

will show the progress you will be recommending to the worker’s family physician (Nominated

Treating Doctor NTD).
