Provide a detailed summary of the target behavior identified in the “Resource file”, including an operation definition of the behavior, a description of the frequency and severity of the behavior, and a summary of how the presence or absence of the behavior impacts some aspect of your life.
Provide a detailed summary of the origin of the behavior, including a discussion of what was maintaining the presence or absence of the target behavior.
Provide a detailed summary of the comprehensive functional assessment as it relates to discovery of issues related to the presence or absence of the target behavior.
Thoroughly describes the intervention created and the attempts to implement the intervention during the two week period.
Identify ethical issues that arose during the intervention and how those were resolved. Discussion should include identification of factors that might have influenced the intervention but that were not considered initially. Discussion should also include what should have been done differently to insure a greater likelihood of success.
