2.Type a five (5) page summary of the disease or condition chosen. The summary must by typed in Microsoft Word, Times Roman, 12pt, double spaced, 1 inch margins. Note: If you do not have Microsoft Word save the summary in a Rich Text File (rtf).
3.The summary (paraphrased) must be a minimum of five (5) typewritten pages and a maximum of eight (8) pages – 1) Cover Page (not included in the count as part of the five (5) typewritten pages) includes the title of the disease or condition; name of student; course title and date; 2) Work Cited Page (not included in the count as part of the five (5) typewritten pages) – all references used in the summary must be listed on this page.
4.Summary should include the following: 1) Introduction about the disease or condition chosen; 2) Etiology of the disease or condition; 3) Risk Factors; Symptoms/Consequences; 4) Diagnosis/Diagnostic Testing; 5) Prognosis; 6) Method of Prevention (if any); and 7) Summary.
5.The summary must include a minimum of 5 references cited in the latest version of the APA Style of Writing within the summary and no reference should be older than 5 years.. These references must come from reputable journals, articles or books. No websites are to be used for this project unless the journal articles are housed on the website. Do not use Wikipedia – this is not an acceptable academic website and do not use your textbook as a reference either.
