COMMUNITY ASSESSMENT INSTRUMENT-This assignment will be done as
part of the clinical component but the paper will be graded as part of your classroom
work.
Students working in teams of no more than 4-6 students will use the following
Community Assessment Instrument to assess a community of their choice. After
collecting the data, students will work together to develop a nursing diagnosis. This
assessment should be approximately 25 pages, typed and which will include
documentation to support each area identified, (these might include photos, census
data, newspaper articles etc) Use APA style for documentation.

DESCRIPTION OF THE COMMUNITY: Using Census.gov and Fact Finder,
select a Census Tract or zip code in order to determine the boundaries of the
community.

inequality in who obtains care?
3. How do health care providers (HCPs) define the type of care that is provided to the
community? How is the care evaluated? 4. How are standards of care set? 5. Is there
an interdisciplinary Care Plan? Which disciplines are involved in this?
