Prepare a report critiquing the incident. Clearly outline the events and discuss the reasons for the accident. Identify the failings that occurred and produce a risk assessment that should have been in place (state any assumptions that you feel that you need to make). Discussing, with reference to legislation, the causes, the outcome and details and conclusions of any investigation surrounding the accident.
Prepare an academic poster to disseminate the information.
